Home Care Clinical — Encore® Clinicals
The ease of use of our homecare clinical software allows clinicians to focus on the patient without sacrificing the quality of their documentation.
Clinicians start to recognize the benefits upon entering the homecare clinical software as they are presented with their work list - identifying what is scheduled for that day, work still in process and work that was due previously but has not yet been completed.
With the ability to graphically document pain assessment and wound documentation on anatomical silhouettes, clinicians can access the existence, nature and intensity of pain or staging of the wound and record the results in a way that facilitates regular assessment and follow-up.
Clinicians can perform consistency checks between OASIS questions and activity data as well as between OASIS and risk assessment questions - further reducing quality assurance time and improving the quality of clinical documentation.
Our user-defined risk assessment functionality allows for weighting and a low, medium, high score range. This enables a homecare provider to identify the risks applicable to their unique patient population and implement the steps necessary to mitigate those risks when applicable.
Our care plan model utilizes a problem based approach to care, allowing for better clinical outcomes scoring. Problems are automatically added to the proposed problem list for a patient care plan based upon the level of risk resulting from our risk assessment functionality.
Advanced Beneficiary Notices within our homecare clinical software allows clinicians to create Advanced Beneficiary Notices from the point-of-care. Combined with our Electronic Signature feature, clinicians have the ability to electronically capture a patient's signature at the point-of-service.
Encore is a tool that positively impacts patient care, clinician satisfaction and efficiency.
